COVID-19 Vaccination Notification Form for Healthcare Professionals

PLEASE NOTE: This form is for those who are not able to administer COVID-19 vaccinations.

The County of San Diego is following federal and state guidelines and working with the County’s COVID-19 Vaccine Clinical Advisory Group to determine vaccine allocation. This form should be completed by healthcare individuals/facilities who are not able to administer COVID-19 vaccinations. This form will assist the County in ensuring vaccination opportunities are available to your organization. Information provided below will be utilized to notify you when you/your healthcare facility is eligible to receive COVID-19 vaccinations from the County of San Diego. Each organization should only submit one form per facility. If you are an individual Psychologist who are frontline/direct contact are able to register on the website as well.

SD Health Department 

All psychologists and any mental health providers in direct contact with patients/clients qualify for Phase 1A happening now. You do not have to register as an organization and individual psychologists who are frontline/ direct contact are able to register on the website below.


Also Kaiser Permanente is giving vaccines to health care providers and you don’t have to be a KP member. You call 800-290-5000. Ask for primary care. They will  register you and give you a Medical Record Number (MRN.). You can access your medical record with your phone number. They will contact you in 3-5 days for an appointment.  It may be more accessible than  going to the county vaccination clinics.
